Usual, Customary and Reasonable (UCR) Programs pay a set percentage of the dentist’s fee or they pay the insurance company’s “customary” or “reasonable” fee limit, whichever is less.

Table or Schedule of Allowance Programs provide a list of what particular dental services are covered along with a price which represents how much your dental insurance will pay for that treatment or procedure. As the patient, you are responsible for paying the difference between the covered amount and the full price for service.
